Please check your appointment book carefully to keep track of appointment times. We recommend arriving at least 15 minutes before your appointment, because it may be necessary to gather and verify information. Make sure you bring all insurance information with you to your first visit.
For certain disciplines, you might need radiographic imaging before you visit. This might require arriving even earlier.
In some cases, we ask you to complete additional medical forms or questionnaires before your get here. It is important to bring these completed forms. Also, if requested, make sure you bring any medical records from other institutions and/or imaging studies, for example, MRI or CT scans, X-rays, etc., with you to your first visit.
If you are coming to the Institute for an outpatient procedure or treatment, we will give you instructions about arrival time, where to go, how to register, and any necessary special preparation. Depending on what procedure or test is scheduled, you might be asked to have a family member or friend drive you home.
